Deer Trail Assisted Living is an inviting community made up of two neighborhoods – Assisted Living and Memory Care for those experiencing Alzheimer’s or other Dementias. Memory Care Assisted Living provides personal care in a residential setting for people who are unable to live independently and need assistance with daily activities, such as bathing, dressing, eating and medication management along with extra care and over-sight due to memory issues.

We serve seniors who are in need of assistance with activities of daily living, medication management, meals, housekeeping/laundry, transportation, meaningful activities, socialization & community. The Wyoming Pioneer Home is a comforting, resident-centered alternative for a senior who doesn't require intensive, skilled nursing care. Assisted living at the WPH is designed for senior adults who wish to maintain their independent lifestyle but may need just a little additional assistance with some daily activities. Everyone is encouraged to stay active for as long as possible. WPH employs a full-time activity director. Several activities are arranged daily, including outside excursions.

New Horizons Care Center is located at the foot of the Big Horn Mountains in Lovell, Wyoming. The facility has 85 beds for long-term care and features an Alzheimer's Unit, skilled beds, intermediate care and Assisted Living. The Care Center is affiliated with North Big Horn Hospital District. With the Care Center and hospital conveniently joined, each resident has quick easy access for medical help. At New Horizons Care Center your loved one will feel right at home

Mountain Plaza provides personal care in a residential setting for people who are unable to live independently and need assistance with daily activities such as bathing, dressing, eating and medication management. Mountain Plaza has state-regulated supportive care and a 24/7 wellness staff.

We work with persons of all age groups and all types of disabilities to live independently and overcome barriers. This includes but is not limited to: Modifications to your home, Rights and Benefits Counseling, Transporation, Case Management for In-home Care, Access to Recreation and ADA Consultation.
