The Children's Mental Health Waiver is a wraparound service that provides care coordination services to help support the entire family. ABLE is an agency with the Children's Mental Health Waiver. We provide Family Care Coordinator, Family Support, Respite, Youth Support, and Youth/Family Training.

I provide DD and CCW case management, supporting people with disabilities to live as independently and meaningfully as possible. I work alongside each person to understand their goals, coordinate services, and navigate systems, while advocating for their choices, safety, and full participation in their community.

Case management is a required service on all Waivers to assist participants in gaining access to needed Waiver and other Medicaid State Plan services, as well as medical, social, educational, community supports and other support services, regardless of the funding source.
