It started as an agency that offered Case Management Services to adults and children with Developmental Disabilities or brain injury under the Developmental Disability Waiver. We offer various programs to enhance and encourage healthy living habits and stimulate mental well-being; opportunities to improve physical strength, flexibility and balance. Exercise classes and physical programs are available. Lunch served 7 days a week and breakfast 5 days a week. The Center also has a lending library and a small gym. We offer a great variety of senior outings throughout the month.

The Home and Community Based Services (HCBS) Section oversees HCBS waiver programs for people with intellectual and developmental disabilities, and acquired brain injuries. We strive to provide excellent customer service and up to date information to participants, families, providers, and other stakeholders.

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.
