Our Intensive Case Management Program supports a limited number of qualified individuals and/or families who meet 125% of the federal poverty guidelines. Approved participants of Project Hope will enter a commitment. Each obligation will receive intensive case management, including but not limited to achieving and/or maintaining housing; enhancing life skills; addressing individual needs; building social skills and community relations, and developing goals to achieve an improved quality of life.

Home healthcare services available. Home Makers and CNA's to assist. Assist with coordination of services, various tasks (i.e. budgeting) determined upon intake. Person needs to have 2 medical conditions to qualify. (i.e. Uses walker and is on oxygen) Congregate meals served in Greybull site and Home delivered meals. Public transportation. Client advocate services for Medicare and associated help.

Recovery support to help citizens in building long-term recovery from addictions. Services provided include referrals, recovery coaching, computers for use, overdose prevention, opioid reversal prevention, all recovery meetings and a safe place to be and meet new friends. Housing First offers case management and employment assistance. Recover Wyoming also offers Telephone Recovery Support (TRS) Program.

MHICM provides Case Management services to veterans and their families to strengthen placements in community settings and decrease the occurrence of re-hospitalization. CCSS is a non-profit organization providing senior programs and assistance for individuals 60 years of age and over. Funded by state and federal grants along with assistance from Crook County Commissioners, Towns of Sundance, Hulett, and Moorcroft. We offer a Nutrition Program, Senior Companion Assistance, Medical Alert, Respite Care, Personal Care, Homemaking Services, AARP Driver Safety Classes, Social Activities, Blood Pressure & Toenail Clinics, Transportation and more!

The goal of Skilled Home Health is to provide treatment for your illness or injury, to teach you how to manage chronic long-term disease conditions and to help you maintain the highest level of wellness while living at home. Our professional staff will work with your physician to develop your plan of care.

The Heptner Cancer Center at Campbell County Memorial Hospital provides advanced facilities and comprehensive treatment programs, including Chemotherapy and Radiation Therapy for patients in the Northeastern Wyoming region. We offer CCH Rehabilitation Services for cancer rehabilitation to help with swelling, mobility, and pain.

First Stop Park County Help Center is a program of the Park County Poverty Alleviation Coalition and Heart Mountain Free Clinic (HMFC). We are dedicated to helping people living in crisis in Park County. Our goal is to ensure that immediate needs met so that they can begin to build towards stability, financial independence, and achieving their goals.

Youth and family support to help with intensive care coordination school, counselors, DFS and probation for families with children who have a behavioral health condition, who need more support, or are risk of out of home placement.

The goal of the ADRC is to raise visibility about the full range of available options; provide objective information, advice, counseling and assistance; empower people to make informed decisions about their long-term services and supports; and help people access public and private programs. ADRCs provide unbiased, reliable information and counseling to people with all levels of income.

Wellness outreach includes mental health counseling, acupuncture and healing body work, case management, and Veterans only AA.

Cheyenne Regional Medical Center’s Transition Across the Community Team (TACT) program is a no-cost, patient-centered program designed to improve quality of life, reduce re-hospitalization and contain costs for patients who are 18 years of age and older and considered high risk. If you’re experiencing an episode of illness, you may receive care across multiple settings—often resulting in incomplete or confusing information.

LIV Health offers case management services to Southeast Wyoming. We can assist with SS Disability, Medicaid, Medicare, and other resources and supportive services. Now offering Case Management Intake Walk In Clinic on Tuesdays and Thursdays 9am-1pm.

MS Navigators — highly skilled, compassionate professionals — connect you to the information, resources and support needed to move your life forward. These supportive partners help navigate the challenges of MS unique to your situation.

We provide court supervised substance abuse counseling, supervised probation and case management services to qualified participants in Park County.

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.

Case management is a required service on all Waivers to assist participants in gaining access to needed Waiver and other Medicaid State Plan services, as well as medical, social, educational, community supports and other support services, regardless of the funding source.

Life management services including guardianship for disabled individuals in Wyoming who are over 21 years of age, power of attorney services, and care management for all ages..
