Case management is a required service on all Waivers to assist participants in gaining access to needed Waiver and other Medicaid State Plan services, as well as medical, social, educational, community supports and other support services, regardless of the funding source.

Recovery support to help citizens in building long-term recovery from addictions. Services provided include referrals, recovery coaching, computers for use, overdose prevention, opioid reversal prevention, all recovery meetings and a safe place to be and meet new friends. Housing First offers case management and employment assistance. Recover Wyoming also offers Telephone Recovery Support (TRS) Program.

PROJECT HOPE INTENSIVE CASE MANAGEMENT PROGRAM
Our Intensive Case Management Program supports a limited number of qualified individuals and/or families who meet 125% of the federal poverty guidelines. Approved participants of Project Hope will enter a commitment. Each obligation will receive intensive case management, including but not limited to achieving and/or maintaining housing; enhancing life skills; addressing individual needs; building social skills and community relations, and developing goals to achieve an improved quality of life.

Partners of CommuniCare can access a CommuniCare Navigator via referral through the CIE platform. CommuniCare Navigators can intake referred clients, conduct screenings, make referrals, & offer case management services to help higher-need individuals meet their needs and set and achieve their goals.

We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.
