The goal of the ADRC is to raise visibility about the full range of available options; provide objective information, advice, counseling and assistance; empower people to make informed decisions about their long-term services and supports; and help people access public and private programs. ADRCs provide unbiased, reliable information and counseling to people with all levels of income.

The Wyoming Home Services (WyHS) program is used to provide services to those qualified individuals who are at risk of premature institutionalization. These services are designed to keep people in the least restrictive environment for as long as possible. Cheyenne Regional Medical Center’s Transition Across the Community Team (TACT) program is a no-cost, patient-centered program designed to improve quality of life, reduce re-hospitalization and contain costs for patients who are 18 years of age and older and considered high risk. If you’re experiencing an episode of illness, you may receive care across multiple settings—often resulting in incomplete or confusing information.

We can assist in taking Energy Share of Wyoming applications when applications are open. When funding is available Utility Assistance, Rental Assistance, Food by Choice Pantry. On Occasion we also assist with: Diapers, Wipes, Hygiene, Toiletries, Adult Diapers, Bed Pads.

Case management is a required service on all Waivers to assist participants in gaining access to needed Waiver and other Medicaid State Plan services, as well as medical, social, educational, community supports and other support services, regardless of the funding source.

Our Intensive Case Management Program supports a limited number of qualified individuals and/or families who meet 125% of the federal poverty guidelines. Approved participants of Project Hope will enter a commitment. Each obligation will receive intensive case management, including but not limited to achieving and/or maintaining housing; enhancing life skills; addressing individual needs; building social skills and community relations, and developing goals to achieve an improved quality of life.

The Heptner Cancer Center at Campbell County Memorial Hospital provides advanced facilities and comprehensive treatment programs, including Chemotherapy and Radiation Therapy for patients in the Northeastern Wyoming region. We offer CCH Rehabilitation Services for cancer rehabilitation to help with swelling, mobility, and pain.
