This dedicated and compassionate group is made up of cancer survivors, family members and generous community volunteers.

Transportation to and from medical appointments

The Transportation Department at Wind River Family and Community Health Care believes in providing safe and secure transportation services to our community. We provide transportation for our patients to their appointments within our facilities and to outside provider-referred specialty clinics. We are here to help with providing consistent and reliable transportation services with excellent customer service.

We provide a wide range of services designed to meet the diverse needs of our clients. From everyday assistance to specialized care, our team is here to bring comfort, dignity, and peace of mind, especially for rural communities and our honored veterans in Colorado and Wyoming.

We provide Home Care services in Sheridan, Johnson, & Campbell Counties. We help people in our communities with daily living tasks (like cooking, housekeeping, showering, medication reminders, dressing, running errands, appointments, etc.) so everyone can live a longer, fuller life in their own home. We can also assist with home care following surgeries, provide respite care for family caregivers who need a break, or just provide an occasional drop-in service to ensure everything is going well.

We work with persons of all age groups and all types of disabilities to live independently and overcome barriers. This includes but is not limited to: Modifications to your home, Rights and Benefits Counseling, Transporation, Case Management for In-home Care, Access to Recreation and ADA Consultation.

Travel assistance benefits are funds that are intended to assist Medicaid clients with transportation costs. These funds are only meant to assist clients to get to and from medically necessary appointments and are not intended to cover the full expense of the trip. When the client meets certain requirements, they may request assistance with mileage or another method of travel. The transportation method is based on the client’s medical needs and cost effectiveness.

Personal Care helps with Bathing, Dressing, Showering, Toileting, Incontinence Care, Memory Care, Medication Reminders, Grooming, Meal Prep, Feeding, and Fluid Reminders. Respite services allow the primary caregiver some much needed time away to recoup and reset. We provide transportation to and from appointments, running errands, or any other needs you have.

Platte Paratransit is a nonprofit organization in Platte County, Wyoming, serving Wheatland, Guernsey, and Chugwater. We provide transportation to medical appointments for people who are elderly, disabled, and otherwise unable to travel. Getting to dialysis in Cheyenne, chemotherapy in Douglas, and orthopedic care in Laramie are just some of the needs we've helped to fill.

The Cody Senior Center is open to anyone aged 60 or older. Membership is free! All you need to do to become a member is fill out a two-page questionnaire. That questionnaire must be updated annually.
