Offering support services for individuals with disabilities

The Children's Mental Health Waiver is a wraparound service that provides care coordination services to help support the entire family. ABLE is an agency with the Children's Mental Health Waiver. We provide Family Care Coordinator, Family Support, Respite, Youth Support, and Youth/Family Training.

Treasure Abilities in Cheyenne, WY, was born in 2016. It started as an agency that offered Case Management Services to adults and children with Developmental Disabilities or brain injury under the Developmental Disability Waiver. In 2020, we expanded our journey to the Home and Community Based Waiver Services.

Offering support services for individuals with disabilities

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.

Offering services to include the National Family Caregiver Support Program, Wyoming State Health Insurance Information Program, Senior Medicare Patrol Program and the Foster Grandparent Program.

Day habilitation services for adults with intellectual and developmental disabilities who are participating in HCBS Medicaid Waiver services through the Wyoming Department of Health, Developmental Disability Services division. We also offer many private pay options through these services. Individuals in our day services focus on activities designed to support them in building daily living skills.

Medicaid helps pay for healthcare services for children, pregnant women, families with children, and individuals who are aged, blind or disabled who qualify based on citizenship, residency, family income, and sometimes resources and healthcare needs.

ABLE provider case management services and traditional and self-directed services to youth and adults who are on the Home and Community-Based Services Waiver. ABLE provides the following services through the HCBS Waiver adult day services, case management, child habilitation services, community living services, community support services, respite services, transportation services, and supported employment.

Wraparound differs from many service delivery strategies, in that it provides a comprehensive, holistic, youth and family-driven way of responding when children or youth experience serious mental health or behavioral challenges.

Home Health We offer friendly, Registered Nurses, Occupational Therapists, and Physical Therapists that provide the best in skilled services. We match these amazing licensed and certified caregivers with you for compatibility.

A.C.E. is a small case management agency that prioritizes our client relationships. We will meet with clients at their place of residence. No need to travel to an office. We pride ourselves on being available and accessible to our clients.

We offer three family-style homes and 10 one-bedroom apartments. Our family-style homes and apartments offer residents: A clean, private, and safe living space, Affordable housing options, Personal choice, Support to improve living skills, Support with meal preparation if desired, Opportunities to do things in the community, and Opportunities to make new friends and spend time with family and current friends.

We strive to provide excellent customer service and up to date information to participants, families, providers, and other stakeholders.
