CASE MANAGEMENT (ALL WAIVERS/ FUNDING SOURCES)

Case management is a required service on all Waivers to assist participants in gaining access to needed Waiver and other Medicaid State Plan services, as well as medical, social, educational, community supports and other support services, regardless of the funding source.

Our Intensive Case Management Program supports a limited number of qualified individuals and/or families who meet 125% of the federal poverty guidelines. Approved participants of Project Hope will enter a commitment.
