The Heptner Cancer Center at Campbell County Memorial Hospital provides advanced facilities and comprehensive treatment programs, including Chemotherapy and Radiation Therapy for patients in the Northeastern Wyoming region. We offer CCH Rehabilitation Services for cancer rehabilitation to help with swelling, mobility, and pain.

The goal of the ADRC is to raise visibility about the full range of available options; provide objective information, advice, counseling and assistance; empower people to make informed decisions about their long-term services and supports; and help people access public and private programs. ADRCs provide unbiased, reliable information and counseling to people with all levels of income.

CommuniCare is a Community Information Exchange® (CIE), a network of service providers who share client information and coordinate care with each other from a centralized technology platform. Partners of CommuniCare can access a CommuniCare Navigator via referral through the CIE platform. CommuniCare Navigators can intake referred clients, conduct screenings, make referrals, & offer case management services to help higher-need individuals meet their needs and set and achieve their goals.

We provide court supervised substance abuse counseling, supervised probation and case management services to qualified participants in Park County.

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. Each obligation will receive intensive case management, including but not limited to achieving and/or maintaining housing; enhancing life skills; addressing individual needs; building social skills and community relations, and developing goals to achieve an improved quality of life.
