WYOMING AGING AND DISABILITY RESOURCE CENTER (WYOMING ADRC)

The goal of the ADRC is to raise visibility about the full range of available options; provide objective information, advice, counseling and assistance; empower people to make informed decisions about their long-term services and supports; and help people access public and private programs. ADRCs provide unbiased, reliable information and counseling to people with all levels of income.

LIV Health offers case management services to Southeast Wyoming. We can assist with SS Disability, Medicaid, Medicare, and other resources and supportive services. Now offering Case Management Intake Walk In Clinic on Tuesdays and Thursdays 9am-1pm.
Providing assistance and navigation of the Wyoming State Medicaid Waiver, including referrals, evaluations, navigation, and support.

We provide Case Management for the Community Choices, Comprehensive & Supports, and ABI waivers. We assist with getting individuals approved for the programs by assisting with all related paperwork and community or agency collaborations. We also assist with completing housing applications, SNAP applications, Medicaid and Medicare benefit information and guidance, and by providing referrals to other resources within our community.

Our Intensive Case Management Program supports a limited number of qualified individuals and/or families who meet 125% of the federal poverty guidelines. Approved participants of Project Hope will enter a commitment. Each obligation will receive intensive case management, including but not limited to achieving and/or maintaining housing; enhancing life skills; addressing individual needs; building social skills and community relations, and developing goals to achieve an improved quality of life.

Services provided include referrals, recovery coaching, computers for use, overdose prevention, opioid reversal prevention, all recovery meetings and a safe place to be and meet new friends. Housing First offers case management and employment assistance. Recover Wyoming also offers Telephone Recovery Support (TRS) Program.
