Home Health Agency-Medical and Caregiving/Senior Care services that help people to stay in their home-like settings. This includes caregiving and nursing services, as well as housecleaning, meal preparation, and dementia care assistance. We also provide services in the assisted living centers and nursing homes for 1-on-1 care, safety, and fall prevention monitoring. All services are overseen by nurses.

Memorial Hospital of Carbon County participates in the State of Wyoming’s Department of Health, Aging Division, Community Living sections, Wyoming Home Services program. The program provides services to Wyoming Senior Citizens and disabled adults ages eighteen (18) years of age or older who experience difficulty performing daily tasks on their own. Priorities shall be given to persons who are at risk for premature institutionalization and be in need of the program.

Senior Center Services Uinta Senior Center, Inc. (USCI) serves as a gateway for local aging populations — connecting older adults to vital services in one place, to help them stay healthy and independent. We serve over 214 Seniors per day in Evanston, Bridger Valley, Afton and Kemmerer. USCI offers a wide variety of programs and services.

Community EMS is a patient-centered model using mobile resources to deliver care outside traditional settings, aiming to improve outcomes and reduce ER visits. The program will refer patients to Laramie County resources, conduct home visits, and assess safety. Initial efforts focus on voluntary home safety inspections to prevent falls, reconcile medications, and install or replace smoke and carbon monoxide detectors.

Personal Care helps with Bathing, Dressing, Showering, Toileting, Incontinence Care, Memory Care, Medication Reminders, Grooming, Meal Prep, Feeding, and Fluid Reminders. Respite services allow the primary caregiver some much needed time away to recoup and reset. We provide transportation to and from appointments, running errands, or any other needs you have.

In-Home Personal and Homemaking Services ( Contract RN, does diabetic foot care in Wheatland. In- Home care includes CNA administered bathing, personal care, and respite. Homemaking services include light cleaning and errands). We offer Emergency Response Units. Services for Seniors offers a state-trained expert in Medicare assistance

Help at Home is customized to target the needs of each individual we serve. The Hub supports while maintaining a person’s dignity, respect and independence. Homecare includes helping individuals with things such as cooking, shopping, essential cleaning, and essential laundry. Personal Care includes assisting with bathing, grooming, feeding, ambulating, exercising, oral hygiene, and skincare.

Originally designated to serve the community’s senior population, in 2009 with construction of our new facility, services and amenities were incorporated to address the needs of the entire community. Now, the Center offers programs and services for residents of all ages; child day care, education, outreach, senior meals, in-home health services, and public-use space for meetings and events.
