COMMUNITY LIVING PROGRAM

I-Reach 2, Inc. provides a variety of community living supports and services. Services and supports are delivered in small group settings of 4 people or less. Each service is tailored to fit the needs of the individual receiving services, whether it is 24-hour support or minimal assistance. We promote an active lifestyle and offer individuals access to the YMCA, WAC and the Warehouse.

We provide Home Care services in Sheridan, Johnson, & Campbell Counties. We help people in our communities with daily living tasks (like cooking, housekeeping, showering, medication reminders, dressing, running errands, appointments, etc.) so everyone can live a longer, fuller life in their own home. We can also assist with home care following surgeries, provide respite care for family caregivers who need a break, or just provide an occasional drop-in service to ensure everything is going well.

In Home Assistance
Our Older Relative Caregiver Program is designed to assist older caregivers (55+) who are raising relatives either under the age of 18 or under the age of 59 and living with a disability. The program’s mission is to instill a sense of hope, accomplished by linking families to community resources; helping in required document completion; hosting monthly support group meetings with provided caregiver respite; and offering a variety of supplemental services.

Personal care services are a 1:1 service provided to assist participants in completing task they would do normally if they did not have a disability. This may include activities of daily living such as bathing, dressing, personal hygiene, etc. Assistance provided may be in the form of hands-on, cuing or prompting. Personal care services are provided in the participant’s home or on their property.
LIV Health offers Veteran-directed Home and Community-based Services, VDC, is a program funded by the VA that allows eligible Veterans to receive services in their home and community while managing their own service plan and budget. Also known as VIP, the Veterans Independence Program helps veterans to live independently, in their own homes, and not in nursing care.
GoGoGrandparent aids older adults and disabled people, enabling independence. Services include transportation, meal and grocery delivery, medication support, and home maintenance. Clients call 855-464-6872 or visit www.gogograndparent.com for tailored assistance like Uber rides or DoorDash meals. A sliding scale membership accommodates lower income. No smartphone required.

Home Instead offers in-home, non-medical care services for seniors in the Wyoming Counties of Albany, Converse, Laramie, and Natrona. We offer care services such as personal care, transfer assistance, light housekeeping, meal preparation, medication reminders, and transportation to appointments. Clear Creek Wood Bank was formed in order to provide a supplement heat source to those who heat their homes with wood and are unable to afford or physically perform the act of making firewood. We coordinate with foresters, a local timber business, and private forest landowners to collect, cut, and give firewood that has been sustainably harvested from the Bighorn Mountains. We provide a limited amount of firewood once a week, until we run out for the winter.
